District Policy Guidelines regarding medication at school

All medicine brought to school must accompany the proper district form before office staff can give ANY medication to your child.
Over the counter medicine includes cough drops, cold medicine, tylenol, ibuprofen, etc.   District policy states we cannot give students any herbal medication.

Prescription medicine must be brought to school by the parent (not sent in a backpack) and must also have the proper district form filled out by the parent AND signed by the physician.  Peace Health Pediatrics also has these forms and can fax to us.

To download a from from our website go to Health Information on the menu bar and click Online Forms.
